Cause Description:
The HCN was founded to explore options and identify pathways for integrated support in a smaller geographic region that is already a well established community. We have identified five priority areas of community living: employment, residential options, recreation, health care and transportation. We look to partner with service agencies, direct service providers, area businesses, our local school district and any other interested parties to connect individuals with developmental disabilities and their families to the supports they need across all the priority areas. In this way we can help all families in this region work together to meet the needs of all our local residents with DD.
